Every Summer, New Haven Farms offers our signature Farm-based Wellness Program – a program designed to prevent the onset of diabetes and other chronic, diet-related diseases. Our program is 16-weeks in the summer and based off of the CDC’s diabetes prevention program (DPP) that has been shown to prevent the onset of diabetes better than metformin and has been shown to be effectively translated to the demographic in New Haven.
We work with local providers and health clinics who refer their patients to our program.
To make a referral:
- Patient must have risk factors for diabetes, prediabetes or diabetes and be at or below 200% of the poverty line.
- Our programs are offered in English and Spanish in both the Fair Haven neighborhood and the Hill neighborhood.
… If you are a Fair Haven Community Health Center employee – you can make a referral internally through EPIC, the referral is called “New Haven Farms”
…If you are a Yale Primary Care or Saint Rapheal’s employee you can send a message in EPIC to Elizabeth K Marsh with the patient’s chart and risk factors
…If you are a Cornell Scott Hill employee you can refer your patient by getting verbal consent from the patient and emailing firstname.lastname@example.org with the patient Name, DOB, Phone and risk factors or diagnosis.
New Haven Farms uses encrypted software to store patient information and asks all new participants sign a form consenting to NHF staff communicating with their primary medical practitioners about relevant health information (changes in weight, diet, etc.).
